Access and Account Settings

How do I get started?

To get your company started or to find out more information about myPersonalWellnessGuide, either contact your company broker or consultant, or contact us directly at info@selfhealthnetwork.com, or call 415 477-9908.

If your company has already supplied you with a username and password, watch this video for a quick introduction.

What do I do if I forget my password?

Simply request a new password using this form. Follow the instructions on that page by entering the e-mail address that you used to sign up for your account, and a new password will be sent to that e-mail address. When you log in, you can change your password by clicking on "Account Settings" on your profile page and choosing the "Password" link.

How do I change my password?

After logging into your account, go to the "Account" link at the top of the page and choose the "Update Password" link. Follow the instructions.

How do I update/change my email address?

After logging into your account, click the "Account" link on your Home page. Enter your new email in the email field and submit it.

How do I update/change my user profile?

After logging into your account, click the "Account" link on your Home page. Enter your new desired user profile information and save it.

What is a Secret Question & Answer and why is it necessary?

A Secret Question & Answer is an extra security measure provided to protect your account. It serves as a way to prevent unauthorized users from attempting to access your account or change your password.

Are my answers, journals, and medical records private?

Your access is completely confidential. No one will see you information unless you choose to share it. Your nutrition and fitness plans do have social sharing that will allow you to keep your friends informed of your progress. If you want your doctor to see your health records, you need to give them access.

Medical Records

How do I add files into my medical records?

You will receive your own, personal telephone number when you join. This number can be used to receive both faxes and voice messages. This number is also your user ID. Every document faxed to your number or voice message left becomes a new record. You can get a series of "authorization stickers" on the Forms page. You give the form(s) to your doctor(s) so that they can fax in your medical records upon your request.

Can I upload images?

Yes! You can upload images such as medical reports, x-rays, scans, and MRIs into your account directly from your computer.

What happens when my records are faxed into the system?

Each time a document is faxed to your account, MPWG will send you a notification (to up to three e-mail addresses that you specify when you enroll). These e-mail addresses can be on your computer, on a text-enabled cell phone, or on a text pager. The next time you log in to your account, you''ll see alerts for new documents in your "Message Center."

How do I view my records?

Next to each new record is a "view" link. Click on this and a new browser window will open with your document shown in Adobe .PDF format.

How do I file my records?

MPWG provides you with 16 file folders for categorizing and storing your records. You may want to categorize your files by different types of medical specialties and events, such as hospitalization or surgeries. To file your records, simply click on the "file" link and a form will appear that lets you choose as many different folders as you want into which you can file your records. You can also add information such as doctor name, and date to the record, as well as your own comments.

How do I find my records after they are filed?

Each time you access your account, you will see all of your records listed by clicking on the "My Medical Records" button in the navigation bar. Your records can be sorted by title, by date, by doctor, or by family member. Click on the "view" link and you can see your records; click on the "edit" link and you can change information about the record. You also can locate records by using our free-form search feature.

Can I store non-medical information on the site?

Yes. You can store any information you choose on the site. In fact, your MPWG account is your own, personal, virtual safe-deposit box for all your critical life records, whether medical records, financial records, insurance policies, contracts, etc. We recommend that your keep a copy of your driver''s license and passport stored in your account so you can quickly re-establish your identity in the event these documents are lost or stolen.

Can I print out my records?

Yes. When you view your records, you can print them or even save them to your computer''s local drive.

Can I give someone else access to my information if I choose?

Yes. The information is yours and you can do with it as you please. You can supply your physician with an emergency contact number so he/she can access your emergency records if an emergency arises.

What is my Emergency Password?

Your Emergency Password is a special password you create that allows medical personnel to access selected information in your account in the event of a medical emergency in which you can''t communicate. You decide what information you want to be displayed in the event of an emergency. When you enroll, you receive a special sticker that you can put on the back of your driver''s license or other identification. We recommend that you write down your User ID and Emergency Password on this sticker. In the event of an emergency, medical personnel would access your account through the "Emergency Login" button on the home page.

Can I delete information from my medical records account?

Yes, you can delete any document. However, please bear in mind that once a document is deleted, it cannot be recovered.

Fitness and Nutrition

When I post a message or comment in the Community section of Fitness and Nutrition, it displays the wrong time. How can I fix it so it shows the correct time?

The system is set up with a default setting displaying the Eastern time zone. You can easily adjust your settings for the proper time zone.

  • Go to the Account Settings page in the Fitness and Nutrition part of the site.
  • Under "General Site Preference," select "Time Zone."
  • Use the drop-down menu to choose the correct time zone for your location and submit it.

How many Workout Plans can I do at one time?

You can only do one workout plan at a time. However, you can easily change the plan to meet your needs. Use the "Add an Exercise," "Delete," or "Swap" links to change exercises and customize your workout plan.

If I am unable to log on and submit my exercises for a few days, can I still add them in at a later date?

Yes, you may submit exercises for days in the past. Just use the calendar to navigate to the day(s) you want to track, enter your exercises, and submit. Note that workouts submitted beyond two weeks in the past will not be awarded points.

I found a Workout Plan I like, but it is only a seven-day plan. How do I keep this plan for a longer period of time?

When you choose a plan, it will have a set number of days associated with it—usually 3, 5, or 7 days. Each day that you participate and submit a day’s workout, one more day is added to the workout. You never have to update it or add days. It will automatically renew the original number of days.

How does the Workout Tracker differ from a Workout Plan?

You can use the Workout Tracker to enter individual exercises you do each day. The "Add an Exercise" button will give you a drop down of all of the exercises and you can select one from the menu. If you want to customize an exercise you can do so by creating a new exercise using the "Create A New Exercise" button and pulling in the exercise(s) of your choice. If you do an exercise today and wish to copy it to another day, you can use the "copy day" button to copy your exercise(s) to the current day. You will choose to enter reps, lbs, time, or distance as the proper measurement of the exercise; this allows the system to track the calories you burn. If you like your exercises, you can choose the "Save as new plan" link and fill in the details about your custom plan.

I want to change some of the exercises in a plan. How do I do that?

You can use the "swap," "delete," and "add an exercise" links for those days when what you planned doesn''t match up with what you want to do or what you actually were able to do! When you elect to use a Workout Plan, remember to submit your workout to get more planned workout days added to your schedule. Also, if you try out a workout plan for a while and decide you want to try a new workout plan or switch to tracking only, just click the "switch" button to select a new workout plan or go back to track only mode.

What are points and how do I earn them?

Health points are a great way to challenge yourself and your peers to participate in your wellness plan. You receive points for all kinds of activities related to Fitness and Nutrition. Read this explanation of how points work.
